Why a Transition of Care Seminar
This seminar is designed to help family physicians, internists, OB/GYNs, pediatricians and clinical healthcare professionals to understand and address needs of special needs patients by developing transition policies for their practice.

The American Academy of Pediatrics – Georgia Chapter designates this live activity for a maximum of 6.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
This activity will be submitted to Georgia Nurses Association for approval to award contact hours. Georgia Nurses Association – Approver is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation. A maximum of 6.75 contact hours will be submitted for approval.
Upon completion of this course, the attendee will be able to:
- List core functions of transitioning youth from pediatric to adult primary care
- Discuss resources available to support transitioning youth from pediatric to adult primary care
- Identify aspects of successful transitioning youth from pediatric to adult primary care
- List waiver programs available for youth

The conference will be held at the Georgia AAP headquarters located at 1330 West Peachtree Street NW, GA 30309.
Parking is free in the parking decks underneath the Building. Paid parking is available in the surface lot next door for $10.00 per car or more. Please have your License Plate Number on hand to enter it into the parking machine. Credit Cards and Cash are accepted.
