Why a Transition of Care Seminar

On March 25, 2017, the Chapter hold a conference on Transition of Care.  This seminar is designed to help family physicians, internists, OB/GYNs, pediatricians and clinical healthcare professionals to address issues surrounding transitioning youth with special health care needs from pediatric to adult care.

Who Should Attend

This seminar is open to all family medicine physicians, internists, OB/GYNs, pediatricians, physician assistants, residents, medical students, nurse practitioners, nurses and other healthcare & industry professionals.

  • Describe policies and resources to support transitioning youth to adult care.
  • Recognize the parents and young adult’s perspective in managing the transitions to adult care.
  • Recognize the need to support transitioning youth with special health care needs to adult care.
  • Describe how to prepare a Transition of Care Policy for your practice

Meeting Location & Parking

The conference will be held at the Georgia AAP office conference center in the Arthritis Foundation Building located at 1330 West Peachtree Street NW, GA 30309.

Parking is free in the parking decks underneath the Arthritis Foundation Building. Paid parking is available in the surface lot next door to Arthritis Foundation for $10.00 per car or more. Please have your License Plate Number on hand to enter it into the parking machine. Credit Cards and Cash are accepted.

Hotel Information

The Artmore Hotel is located at 1302 West Peachtree St, Atlanta, Ga 30309, and is just two doors down from the Arthritis Foundation building.  The rates start at $144 per night.  Call the Artmore Hotel at 404-876-6100 to make a reservation and ask for the Ga Chapter AAP rate.  Be sure to make your reservation by March 10, 2017 to receive the group rate.
