 Additional Information from the Georgia VFC Program Regarding 2015-16 Influenza Vaccine Ordering

 

VFC(Below is a follow up message from the Georgia VFC Program regarding the 2015 influenza vaccine pre-book survey. Please note that the VFC Program will not accept hand written requests.  All request must be submitted through the survey tool.  Any faxed or e-mailed pre-book requests for the 2015-2016 season will be discarded. Also, please see the Chapter blastfax of January 27, 2015 for additional information.)

The forwarded information below and the attachments will be distributed to VFC providers before COB today (January 28, 2015).  The actual pre-book link was released late Monday afternoon.  This information has been provided for VFC providers to assist them with developing their flu estimates for the 2015-16 flu season.  The PDF is a copy of the survey questions, and the algorithm will help them decide how many doses to order for their 6 months – 8 year old patient population based on ACIP and CDC recommendations.

VERY IMPORTANT***  We will not accept hand written request.  All request must be submitted through the survey tool.  Any faxed or e-mailed pre-book requests for the 2015-16 season will be discarded.

Note:  One attachment is for Private Providers (does not contain SBF pre-book section) and the other is for Public Health clinics (contains SBF pre-book section).
