Georgia WIC Policy Changes Effective October 1, 2021

New Medical Documentation Form & Policy Issuing
Nutritional Supplements for Children & Women

The new documentation form, the Request for Medical Formulas Form (RMF), replaces the Medical Documentation Form (MDF). This new form has a stream-lined look designed for faster & easier completion!

Additionally, the policy on issuing nutritional supplements for children & women will go into effect which addresses documentation requirements for the approval of nutritional supplements like, PediaSure, Boost, and Ensure.  Specific guidance is provided for children that are diagnosed with failure to thrive, oral motor feeding disorders, developmental disorders causing limited food intake, and hypermetabolic conditions.
